Previously, we demonstrated a significant percentage of black youth with a reduced compensatory increase in urinary sodium excretion UNaV ; in response to stress-induced increase in blood pressure BP ; . We refer to this pattern of response as impaired "stress-induced pressure natriuresis SIPN ; . The epithelial sodium channel ENaC ; consisting of three subunits constitutes the rate-limiting step for renal sodium reabsorption. This study aimed to test the hypothesis that polymorphisms of the ENaC genes contribute to impaired SIPN. Two hundred and ten black and white normotensive youths 109 blacks and 101 whites ; aged 16 19 years were recruited. A pre-stress urine sample was obtained prior to testing. The subjects then performed two reactivity tasks, a video game challenge and forehead cold stimulation followed by a post-stress urine sample. BP was obtained throughout the tasks and averaged to obtain a measure of BP that was concurrent with the measure of UNaV. The T594M, G442V, C1990G, A2139G, G173A, T663A, A334T and C618F polymorphisms of the three ENaC subunit genes were genotyped by enzyme digestion assays. Blacks with the A173A genotype as compared to blacks with the A173G and G173G genotypes showed a greater increase in UNaV during stress [9.94 1.02 n 19 ; versus 2.36 0.78 mEq hr n 62 ; , 0.001, R2 0.36]. Unlike blacks, whites showed no relationship between the G173A polymorphism and changes of UNaV P 0.2190, R2 0.03 ; . This differential effect of race was significant P 0.0028 ; . Individuals with the 594M polymorphism had an approximately 50% reduction in changes of UNaV compared to those without [2.40 1.69 n 11 ; versus 4.59 0.57 mEq hr n 98 ; The difference did not reach a statistical significance due to the low frequency of the 594M allele P 0.20, R2 0.01 ; . Renin levels to return to normal, suggesting that the increased urinary excretion resulted from filtration of renin produced by the "ischemic" kidney through the normal left kidney. In four other patients 3, 10, 14, and 15 ; , renin excretion was above the normal range for males, and in three of these patients plasma renin was also elevated. Although in normal subjects independence of renin and protein clearance was observed, it was anticipated that in patients with heavy proteinuria there would be a concomitant increase in urinary renin excretion and clearance. However, this was only the case when plasma renin was also elevated. Thus, patients 6, 11, 12, and 16 had normal renin excretions in spite of heavy proteinuria, while patients 3, 7, 8, and 15 with elevated plasma levels displayed renin excretion above the limits of normal for their sex. Patient 17 was the only exception to this pattern. In this patient heavy proteinuria and elevated plasma renin were not associated with excess renin output and carbenicillin.

Cardiovascular surgical consultation is required in all patients with suspected aortic dissections. Surgery is indicated for all dissections involving the ascending aorta type A dissection ; , with the exception of only a few patients with serious associated conditions contraindicating surgery.136, 137 Patients with hypotension suggesting aortic rupture are candidates for emergency surgical repair. Complications of type B dissections, such as leakage of blood from the aorta, impairment of blood flow to an organ or limb, or persistent pain despite an adequate medical regimen are best treated by surgery. Younger patients with Marfan's syndrome may benefit from surgery in the subacute phase and avoid rupture of a residual saccular aneurysm in the future. Patients with uncomplicated distal dissections are best managed medically in the acute phase with antihypertensive therapy, as survival rate is around 75% whether patients are treated medically or surgically.31 Also, these patients are generally in the older age group, with a history of cardiac, pulmonary, or renal diseases. Hypertension After a Cerebrovascular Accident In healthy humans, cerebral autoregulation maintains constant CBF between a mean systemic arterial pressure of 60 and 120 mm Hg. However, in patients with chronic hypertension, autoregulation is set at a higher level approximately 120 to 160 mm Hg ; , presumably to protect the brain from the effects of persistent hypertension Fig 1 ; .27 After a stroke, the normal mechanisms of cerebral autoregulation are impaired. Perfusion in the ischemic penumbra becomes pressure dependent. A rise in systemic arterial pressure may be an adaptive response to maintain the blood flow to this vulnerable area. In a series of 334 patients with acute stroke admitted to the hospital, Wallace and Levy138 found that 80% had elevated BP on the day of admission. The BPs fell spontaneously and gradually over the next 10 days. By the 10th day after the stroke, only one third of patients remained hypertensive. The mechanisms underlying hypertension after a cerebrovascular accident have not been fully elucidated. Activation of the sympathetic nervous system may be involved as part of a global metabolic response to cerebral infarction, cerebral hemorrhage, or associated edema. There is no evidence that hypertension has a deleterious effect on the outcome of ischemic strokes during the acute phase.32, 139, 140 Lowering the BP in patients with cerebral ischemia may reduce CBF, which, because of impaired autoregulation and carmustine.
In determining the likelihood of confusion, the examiners must take into account all the surrounding circumstances. In the Pianotist Case, reported in 1906 ; 23 R.P.C. 774, Parker J., after reviewing the surrounding circumstances which had been the subject of judicial consideration on many occasions, stated at p. 777: "You must take the two words. You must judge of them, both by their look and by their sound. You must consider the goods to which they are to be applied. You must consider the nature and kind of customer who would be likely to buy those goods. In fact, you must consider all the surrounding circumstances; and you must further consider what is likely to happen if each of those trade marks is used in a normal way as a trade mark for the goods of the respective owners of the marks." In order to consider all the surrounding circumstances, the examiners are directed to subsection 6 5 ; of the Act. The following case provides guidance on its provisions.
